A Simple Wedding Officiant will perform your ceremony at the date, place and time agreed upon and set forth above. It is
understood that time is of the essence and therefore we schedule our time according to the starting time for the ceremony, which
you provide to us. We try to arrive one half hour prior to that time and we allow one hour from that time to perform the ceremony
and take care of any paperwork which may be required.
If the ceremony is more than one half hour late in starting there will be an additional fee which will be payable prior to the
beginning of the ceremony.
We apologize for the need to do this but too many people tend to run very late and many times we have another event which we
must attend.
SUBMIT YOUR FORM HERE ###  Be sure to Submit this form before going to
the Credit Card section. Also if the name on the credit card is other than the
bride & groom please make note of it on the form. Thank you
MAKE YOUR DEPOSIT BELOW.
      PLEASE READ THE FOLLOWING:

A DEPOSIT of $50.00 is required to reserve your date and time. This is a non-refundable deposit but can be
applied to a rescheduled date.

PLEASE ENTER NAMES AND DATE in the blanks before clicking the button below to make your deposit. The info
sheet and deposit come as two separate items and this helps with identification.

BALANCE DUE: In cash day of ceremony

TWO DAYS PRIOR TO DAY OF CEREMONY...Call  Donna at TheSimplewedding 954-254-6216 to confirm all is going
as planned

DAY OF CEREMONY: Please provide license and balance of fee to officiant prior to ceremony. This avoids
confusion after the ceremony.

Scheduled Start Time

Officiant will arrive 30 minutes prior to scheduled start time.

PLEASE BE SURE TO GIVE US THE TIME YOU WISH TO START THE CEREMONY, NOT THE TIME YOU PROVIDE THE
GUESTS.

WE ARE FINDING MANY LATE STARTING WEDDING AND BECAUSE WE SOMETIMES HAVE A FOLLOWING WEDDING
TO CONSIDER...

.If  wedding starts MORE THAN 30 MINUTES late there will be an ADDITIONAL CHARGE OF $25.00 for each 30
minutes or portion thereof.
